Human Error Reduction Systems (HERS)™

Human errors are inevitable part of human life and human performance. They are reflection of our own imperfect design but also the  mechanism which allows us to simplify extremely complex information and recover out of control situations. One cannot exist without the other. 

When organisations design risk management systems and processes, potential for human errors and error promotion conditions are often overlooked in favour of standard, off the shelf safety management systems and procedures. The focus is placed on things such as ‘how’ the job is done, ‘what’ tools and equipment is required, as well as host of other safety and training related information. Risk assessments are performed and the operation goes ahead.

What usually follows over the long period of time is a string of warning signs such as process failures, near misses, injuries, incidents, and sometimes major disasters, where various forms of human errors have combined with systemic, organisational and environmental influences to produce adverse effects. Organisations investigate and often arrive to human error as a 'root cause'. They also often apply heavy disciplinary actions in an attempt to ‘eliminate’ or reduce those errors.  Trouble is, human errors are neither root causes of accidents, nor can they be eliminated. They are not moral or value based issues but rather a matter of a complex interplay between a particular work environment, workplace conditions and factors and cognitive state of people at the time of incident - error awareness.

 

Error awareness is a sub state of situational awareness where people are acutely aware of practical possibilities and scenarios where their intent may not not transpire in appropriate action and have appropriate information and means to ensure their action is a correct one and that it can be confirmed as such.

The only way to minimise occurrence of human errors in industrial environment is to understand how they occur, what people at the sharp end do and why, and build error tolerant systems of work which cater for various forms of human fallibility.
